Blackboard User Account and Course Request Form

Please complete this form to have a user account and/or new course created for you on the Blackboard server. When your account or course is ready to use you will be notified by email.

Please complete a request form for each Blackboard course that you want created. List only the section numbers that you want to include with the specified course. There are instances where multiple Blackboard courses are created for different sections of the same course. For example, suppose an Accounting course has three sections (01,02, and 03). Two of the sections are in Cumberland (01, 02) and the other section is in Somerset (03). The instructor wants separate Blackboard courses for the Cumberland and Somerset sections. To do this, the instructor would complete two request forms, listing the Cumberland section numbers on one form (01, 02), and listing the Somerset section number (03) on the second form.

An example of a course ID number is WE.41.221.1032.2003.FA.  To find the course ID, go to your course control panel and check at the top of this screen.  The course ID number will be displayed here.

When a course copy is done, only course content is copied.  No student/user data is copied from one course to another.  Also, after a course copy is done, the “old” course still remains on the Blackboard server.  No courses are deleted unless the instructor of the course specifically requests that the course be deleted from the system.
